By signing up for the online application form on Food Lane, you acknowledge that you have read and understood the following terms and conditions:

You acknowledge that you have read and understood the Data Privacy Notice provided by Food Lane. By signing up for the online application form, you agree to provide your personal data and business data as required in Form A and Form B. You consent to Food Lane collecting, using, disposing of, and processing your personal information provided in the application form for legitimate purposes as stipulated in the Data Privacy Notice.

You are responsible for providing accurate and complete personal details, business details, and vehicle details during the registration process. You must promptly update any changes to your personal, business, or vehicle details to ensure accuracy and compliance with our platform's requirements.

You must be at least 18 years old to register on Food Lane. Food businesses must comply with all local regulations and licensing requirements to operate legally. Vehicles used for food delivery must meet safety and regulatory standards set by local authorities.

Food Lane reserves the right to refuse service, terminate accounts, or cancel orders at its discretion, especially if there are concerns regarding the safety or legality of a user's operations. You agree to use the Food Lane platform solely for lawful purposes and in compliance with these terms and conditions, as well as any applicable laws and regulations.

Food Lane shall not be liable for any damages, losses, or liabilities arising out of or in connection with the use of its platform, including but not limited to direct, indirect, incidental, consequential, or punitive damages. You agree to indemnify and hold Food Lane harmless from any claims, damages, or losses arising out of your use of the platform or any breach of these terms and conditions.

Food Lane reserves the right to update or modify these terms and conditions at any time without prior notice. Continued use of the platform after such changes constitutes acceptance of the updated terms.

By signing up for the online application form on the Food Lane online platform, you acknowledge that you have read, understood, and agreed to these terms and conditions. If you do not agree with any part of these terms, you should not proceed with using the platform.

List of Requirements:

1. Latest two (2) pieces “1 x 1” ID picture

2. Two (2) photo of truck (front and side view)

3. Scanned Business Permit/SEC Registration/CDA Registration

4. Scanned Mayor’s Permit

5. Scanned Board Resolution authorizing representative to transact business in relation to the Food lane project (for coops, corporations, associations), as it may apply

6. Scanned Authenticated copy of Official Receipt of Registration

7. Scanned Authenticated copy of Certificate of Registration (original copy will have to be presented

8. Scanned Certificate of attendance to the Food Lane Seminar to be issued by DA

9. Scanned Inspection Report of Transport vehicles conducted by DA-AMAS/AMAD

10. Duly accomplished Foodlane Accreditation Application Form (kindly print, sign and scan)

11. Duly accomplished Foodlane Reference Form (kindly print, sign and scan)

12. Duly accomplished Statement of Commitment (kindly print, sign and scan)

13. In case of livestock, poultry and by-products, scanned copy of handler's license and accreditation of transport carriers issued by the Bureau of Animal Industry (BAI)
and accreditation of meat transport vehicle issued by the National Meat Inspection Service (NMIS)
